24-HOUR ON CALL

With our 24-Hour answering services, clients can feel confident that their health matters will be addressed in a timely manner — without sacrificing their normal daily routines.

  • Bathing and personal hygiene

  • Dressing and grooming

  • Assistance with mobility, transfer exercises

Non-Skilled Services

Case Managers

  • Develops a plan with client’s goals

Transportation

  • We register and scheduled all member’s medical transportation through Mart (PT1)

Registered Nurses

  • Evaluate potential clients

  • Plan and implement client’s care plan that best fit their needs

  • Monitor client’s health status

  • Monitor nutrition intake

  • Monthly safety supervision

  • Increase client’s health knowledge

  • Recommendation and referral for additional services
